Heritage Health Care Files Notice of Recent Data Breach – JD Supra

On January 9, 2025, Heritage Health Care filed a notice of data breach with the U.S. Department of Health and Human Services Office for Civil Rights after discovering that information in the company’s possession was subject to unauthorized access. In this notice, Heritage Health Care explains that the incident resulted in an unauthorized party being able to access consumers’ sensitive information. Upon completing its investigation, Heritage Health Care began sending out data breach notification letters to all individuals whose information was affected by the recent data security incident.

The Heritage Health Care data breach was only recently announced, and more information is expected in the near future. However, at this early point, Heritage Health Care’s filing with the U.S. Department of Health and Human Services Office for Civil Rights provides only limited information on what led up to the breach. Moreover, the company does not appear to have issued a press release or posted a website notice discussing this incident.
However, based on the company’s filing with the U.S. Department of Health and Human Services Office for Civil Rights, we know that the breach involved a “Hacking / IT incident” of a network server. The Heritage Health Care breach impacted an estimated 12,162 people.
Importantly, this doesn’t necessarily mean that Heritage Health Care was the victim of a cyberattack, as it is possible that the company is merely reporting a third-party data breach that occurred at a vendor or affiliated company.
Regardless, after learning that sensitive consumer data was accessible to an unauthorized party, Heritage Health Care reviewed the compromised files to determine what information was leaked and which consumers were impacted.
On January 9, 2025, Heritage Health Care sent out data breach letters to anyone who was affected by the recent data security incident. These letters should provide victims with a list of what information belonging to them was compromised.
Heritage Health Care is a provider of home health care services specializing in delivering personalized medical and non-medical care to patients in their own homes. Headquartered in Maumee, Ohio, the organization serves a wide range of clients, including seniors, individuals recovering from illness or surgery, and those managing chronic conditions. Heritage Health Care offers services such as skilled nursing, physical therapy, occupational therapy, speech therapy, and personal care assistance. The organization employs approximately 500 employees and generates an estimated $13.5 million in annual revenue.
